General Information

The office has a 15 minute late policy. If you arrive more than 15 minutes after the scheduled time of your appointment, we may require you to reschedule in order to avoid delaying other patients' appointments. Exceptions may be made at our discretion, depending upon the circumstances and time constraints. All existing patients are required to fill out the Follow Up Questionnaire; you may print out the form prior to your appointment to expedite your visit. Any patient, who has not been in the office since January 7, must complete a new patient registration. All the forms can be printed out and completed prior to the visit for faster service.

Please make sure you always have your actual insurance card (no temporary or print out will be accepted), picture ID, and a referral, if necessary, at the time of your appointment. Failure to provide these documents will result in your appointment being canceled.

You must also inform the office of any change in your information such as insurance, address and telephone number, etc. If new insurance information is not given prior to your appointment, your appointment will be canceled until the office obtains the information. We apologize for any inconvenience.

No backdated referrals will be given.

All patients must have their referral at the time of their appointment.

There is a 72 hours processing time for referral request. Please fill out the form on the website completely and if calling, have the physicians name, address, phone number, fax number, reason for the referral, and your insurance information. Any incomplete information may cause a delay on your request.

There is a 48 hours processing time for prescription request. Please be advised that some of our physicians are not in the office full time. Allow sufficient time to process your request. Call ahead of time before running out of your prescription.

Payment Information

We accept cash (exact change) or check. We must collect your required co-pay at every visit. There is a $35 fee for appointments canceled less than 24 hours from the scheduled time.

Your First Visit

On the first visit you will be required to complete some forms (Initial Questionnaire, Patient Registration, and Financial Responsibility Agreement) regarding your insurance/billing and your personal health history. If you have health records from previous physicians, please bring them with you. You may print and fill out the Registration, Financial Responsibility Agreement and Initial Visit Questionnaire forms in advance to save time. Please also bring a list of any medications you are taking. We suggest that all new patients plan to arrive 15 minutes before their appointment time to allow for processing.

Insurance Information

We accept most major insurances. Please call your insurance company to verify if the physician you would like to see is participating. Patients are responsible for knowing their insurance benefits.

Frequent Questions

What do I need to do if I am scheduled for surgery?

Find out first if you need a preoperative physical exam. Your surgeon's office can tell you this. If so, call to schedule it as soon as you know the date of your surgery. Allow sufficient time for all your laboratory results. This may take up to 2 weeks.

What if I lose my referral?

Either call our office, or submit one via email. Please do not call us from specialist's office requesting a faxed referral. We are unable to comply with such requests. If you lose your referral or forget it, you will need to reschedule your appointment.

How can I get the results of my tests or radiology?

Please be advised that in accordance with new federal laws pertaining to Health Care Information, we are unable to give test results over the phone or by fax. If you need copies of this information, you need to contact the Medical Records Department at (202) 785-2400 x5 and sign a release for these results to be sent to your physician. The office will send your physician a onetime courtesy copy of your records. Any records that need to be send more than once will be subject to a fee.

How can I get a copy of my medical record?

We will make a single copy to send to another physician's office at no charge to you. If you desire your own copy, there is a minimum charge of $30. You must fax the office a written request for your medical record. If you desired to have if sent to another physician, please include the physician’s name, address, telephone number and fax number.

How can I find out what my copay is?

You must call your insurance company.

Why can't I be billed for my copay?

We are required by insurers to collect this amount at the time of the visit, therefore co-pay will be collected before being seen. We only accept cash (exact change) or check.

Why can't I get through faster to the office by telephone?

We make every effort to answer calls as quickly as possible. We get a large number of calls every day and can't answer them all immediately. There are some things you can do to help:

  • Call during the times that are less busy. Busy times are Monday mornings, Friday, and every day first thing in the morning and just after lunch.

  • Use the forms on this website to request for prescription, appointment and referral.

  • Use our voicemail system to leave requests for referrals, prescription refills and appointment.

  • Be sure to provide all of the requested information. For example, don't leave messages that say "refill my pressure pills", or "birth control". We need to know the following:
    • Medication name
    • Dose (mcg/mg/ml etc.)
    • # per day
    • Supply desired (21/30/60/90 days)
    • Pharmacy phone number
    • Your name and Date of Birth
